FINAL FORMS
Bishop Hartley utilizes a paperless system which is more convenient for our families. In addition to most of the back-to-school “paperwork” which can be completed online via Final Forms, Hartley also uses Final Forms for athletic forms and payments.
This year we are requiring a hard copy of the handbook acknowledgement, the technology agreement, and photo release consent form. Forms will be distributed to students during class meetings the first week of school.
If you are a new family, you will have to set up an account for each of your students. Returning families will find the forms for the 2019-20 school year in their existing accounts.

Jim Peters
Athletic Trainer
Bishop Hartley has the pleasure of having a great medical staff on site to take care of the sports teams here. Jim Peters has been the Head Athletic Trainer at Hartley since 2007, worked with Ortho Neuro from 2007-2015, and has been with OhioHealth from 2015 to present.
EXPERIENCE
Assistant Athletic Trainer at Otterbein College, 1995-2007, BS in Kinesiology from The University of Michigan, 1986-1990
M.Ed., in Secondary Guidance and Counseling from Clemson University, 1991-1993
Associates Degree in Physical Therapy Assistant Technology from Kent State University at Ashtabula, 2015-2016
Ohio Health is the official sports medicine provider for the Bishop Hartley Hawks. Team physician, Dr. Donald LeMay is board-certified in both family medicine and sports medicine. His special interests include treating active patients of all ages and athletes at all levels and sports.
EXPERIENCE
Dr. LeMay has served as the team physician for Columbus Ballet Met Dance Company; The Ohio State University varsity men’s and women’s basketball, gymnastics, golf, fencing, cheerleading, dance and wrestling teams.
Was the head physician for the Capital University athletic teams.
Dr. LeMay has published several articles and has received various awards including Physician of the Year from the Ohio Athletic Trainers Association.
OrthoNeuro has partnered with Ohio Health to continue to be the official sports medicine provider for the Bishop Hartley Hawks. Team physician, Dr. Michael Cannone, has provided care to Hartley athletes since 1998. Dr. Cannone is an orthopedic surgeon at OrthoNeuro specializing in sports medicine and joint replacement. Jim Peters, AT has been the head athletic trainer at Hartley since 2007. Bishop Hartley student athletes are considered “priority patients” at OrthoNeuro and will be seen by an OrthoNeuro physician within 48 hours of contacting the office for an appointment.
